"Ghost" Icon

The app itself is easy to use and I like the visual appeal of the reports. However, this latest update has added a "ghost" icon to my iPad. I have the actual icon that opens the application and a second Icon that is an image of itself. I havent been able to delete it and it doesnt respond to touch. Very strange, never seen this before with an app.
